Background
On February 6, 2021, a service technician was replacing ammonia sensors in a large industrial freezer and never came out. While working alone, he became fatally entrapped in a boom lift that did not have the anti-entrapment system that the company had promoted for years as a life-saving safeguard. He died of mechanical asphyxia and was not discovered until the following morning.
This preventable tragedy shows the real cost of treating a life-saving “optional” safety device as an add-on expense. It was a choice to put profit over basic safety.
Challenge
How do you prove to a jury that a two-thousand-dollar safety device would have saved a life?
Solution
Right Way vs Wrong Way animations showed the fatal entrapment beside the life-saving SkyGuard scenario. Dual views captured both the crash sequence and a close-up of the impact.
